Food Villains Notepad

The Food Villains notepad was created with the idea that sometimes you need to give a list of your allergies to someone who is going to be making food for you, like a family member at Thanksgiving or Christmas. This notepad makes it easy to mark your allergies, rip the page off and give it to another, allowing them to see all of your food allergies in one place! At this stage only the 26 original villains are listed on the sheet with the severity scale bubbles next to them for you to mark how allergic you are. The back side of the page has space for you to write any extra notes you need the other person to know.
